I usually improvise all my scenarios following the world I created as scaffolding. This improve style was tough at first but after like 4 years of it I’ve gotten good. The events that occur are super fun and often I’m surprised by it as well. Normally the only thing not improvised is the beginning

I’ve been playing running games pretty frequently (sometimes almost every day), for the last 6 years and I not only almost exclusively use my own content but I’ve almost created a new game. I use many non-my this creatures and create my own creatures as well. I heavily modified magic and changed how sanity worked (often I played with one person, and I couldn’t have them going crazy all the time and getting regularly killed). I crafted a free world filled with things waiting to be discover. For example, I always have the world controlled from the shadows by a man named Kane. He’s the first homohomosapian and also the first Vampire. As vampires get stronger with age, Kane is the strongest possible being. He controls the world with a company called Ergot and often times the players end up running into them.

I’ve written a couple scenarios, like a mental hospital that when the players enter it locks behind them, it has 5 floors and each floor becomes more corrupted and dangerous until the 5th. There is a stone that escaped from the dreamlands and has taken control of the hospital. A huge monster will appear from the slime in front of the stone and if they destroy the stone the monster dies. My players were a very experienced group of investors with great gear and they lost only one person, but they all took a lot of damage
