Two options - cheap and functional, if not "official and published" that I've used are small whiteboard (hand held) with magnets and/or markers, and index cards. I find the index cards (one per character, one for "end of round", and then adding cards for anything magical, etc. as needed) work well in combination with a master listing of PCs and monsters.

Son of a gun. Yes. My search skills suck.

RPTools, a tool I've been using a long time now, has a

Tracker that is completely editable and does what I need.
